This is a documentation/medicolegal auditability concern — practices need confidence that signed notes cannot be revised without a clear, visible record. Requested: either (a) lock signed notes from further inline editing (changes must go through a distinct addendum entry), or (b) a visible in-note log/audit trail showing exactly what was changed, by whom, and when — not just a status badge.
