I've had trouble with the local hospitals poor medication reconciliation: As patients go into the hospital and come out of the hospital, they don't ask the patient or the patient's primary care office for the patient's medications and just go with whatever is in the chart from an in-system doctor from over a year ago. This has led to multiple adverse patient outcomes.

I want to create a credit card-sized piece of paper that can go in the credit card slots of wallets.

On it will be physician's name, clinic name, clinic phone and fax number, patient medication, allergies, and problem list.

I want this to be printable like the face sheet, but pop up with that information in an organized fashion and printable on a credit card-sized piece of paper. Maybe foldable so it can be the front and back of that credit card-sized paper. Specifically, credit card sized and not business card because those never fit in the credit card slots.
