Social History and Family History free text (37120304)

Can we make the social history and family history free-text? I just want to
write “Father passed at 50 from MI”..I dot want to have to search an ICD10
code for a family history. Same for social history. I don’t want to have to
type a question and then answer. I just want to free-type it. Would be helpful
to have a “preventive maintenance” free-text also. When I click to add my own
“other” box where I can free-text this, the formatting does not come through
and instead it is all one run-on sentence. It’s not easy to follow or read.

The diagnosis field now makes it clear you can search for a code or type your own diagnosis or text.
- New placeholder: "Search or type a diagnosis"
- "Add" option: whatever you type shows up as the first option, Add "<text>". Pick it or press Enter to keep your free text. It replaces "No results found" when nothing matches.

This should be the default option. Having to to add ICD codes for family history is really not helpful. We should be able to just free text in that section. Things like diagnosis, age it happened, that family members history is much more valuable than just a random lists of ICD codes. This is basic chart maintenance.
