Make the Vaccines section on patient face sheets easier to read/more compact

The new vaccine update makes giving patients their vaccine records far too busy and difficult to read/understand. I've attached what the old version looked like - clean and organized, vs the new one looks like (and how it takes up way more space than necessary)
