Option 1(Preferred) : Dedicated GLP-1 Flowsheet
One flowsheet remains in the patient's chart throughout treatment.
Weekly entries include:
Date
Weight
Medication
Dose administered
Administration site (if applicable)
Staff initials
Notes/comments
Historical data remains visible and trends can be reviewed easily.
Option 2: Smart Form with Running Log
A single form is created and updated weekly.
Each visit adds a new row to the existing document.
Prior weights and doses remain visible for reference.
Allows quick review of progress without opening previous notes.
Option 3: Template Plus Flowsheet
Continue using a weekly visit template for documentation.
Add a separate GLP-1 tracking flowsheet that carries forward all historical data.
The encounter note captures the visit, while the flowsheet serves as the longitudinal medication log.
Concern with Templates Alone:
While templates can standardize documentation, previous administration data does not automatically carry forward into the next template. This makes it difficult to track dose changes, weight trends, and administration history over time. A persistent flowsheet or tracking form may provide a more efficient solution.
