Automated intake form delivery that is automatically linked to the patient chart.
Description:
When a new patient becomes active (e.g., after paying for a membership), automatically send an intake form in a way that it is assigned to the patient chart so the completed form returns to that patient’s chart and can be mapped into their medical record.
Current limitation:
Today, only sending a form from the patient’s chart creates that chart link; public URLs can be shared, but they won’t map into the chart unless assigned to the patient chart. There are, to our knowledge, no integrations/partners that would enable or allow for this.
Use case:
Trigger on “new patient becomes active” and send the form (or link to Hint-maintained form) via email or SMS from the patient chart.

Additionally the information goes from the form TO the correct hint section. Like when they fill out meds, it goes to the meds section. When they fill out insurance, it goes to the insurance section.
