All-In-One forms - mapped data on a medical history form to the patient's chart

When you send a medical history form in All-In-One from the clinical forms page, the responses can be mapped to the patient's chart so that responses are documented on the patient's chart as structured data. Example: the form asks "what medications are you currently taking?" and the response is then logged on the patient's chart under "current medications"

When a form is created in Clinical and "mapped labels" are used, it does not automatically list the patient's information (except for patient name) when it is sent to a patient.

Currently, patients are not filling out a medical history questionnaire as a part of patient sign-up. This means that the patient's medical history data is not populated into the chart and requires the practice to fill out this information for the patient.

The ability for questionaires to import answers to intake questions (dx, meds, family hx, social, etc) directly into the correct areas in the patient chart. This avoids the added work of putting it in manually.
