Add field to the patient list export or the memberships report export that indicates whether an active member is the primary member (owner) or a dependent

To make it quicker/easier to determine whether a patient is the primary member or a dependent.
Right now, the 'Member Type' field just indicates 'Adult' or 'Child' for retail/self-pay members, and the 'Membership Owner' field must be referenced to see which adult is the primary member.
